What is Chain of Custody?

Chain of custody is the documented, unbroken trail that records who had access to evidence, when, where, and why — from the moment evidence is first identified through to its presentation in court. A single undocumented gap, unexplained transfer, or late disclosure can render evidence unreliable. When the chain breaks, the integrity of the entire case is compromised. This exhibit documents every stage, every requirement, and where failures demand accountability.

Integrity Assessment

A chain of custody with two broken links and two warning-stage failures does not meet the standard required for reliable evidence presentation. Every broken link is a point where truth could have been altered, suppressed, or lost. The public has a right to know.
